Please print all three forms: Statement of Counseling, Privacy Policy, and Application. These forms need to be completed and brought to your appointment. If you wish a telephone session, please send in the completed paperwork to our office (address listed above). Once received, our receptionist will call you to set up an appointment. For phone counseling, please keep a copy of the paperwork for yourself.

Please print the three forms listed above (Statement of Counseling Services, Privacy Policy, and Bankruptcy Letter Application) as well as the two forms listed in this section (Affidavit for Credit Counseling and Bankruptcy Letter). The Affidavit will need to be signed in front of a Notary Public. A copy of your driver’s license (picture ID) will need to be attached. If this counseling session needs to be done over the phone, we will need all completed paperwork with fee sent to us at the address listed above.
